2010 Individual Checklist

  2. Collect your tax preparation documents and check each item that apply to you.

  4. Have your federal and state income tax returns e-filed for fast refunds, get $5 off when you bring the following checklist and all you documents.
 
2010 Individual Income Tax Preparation Checklist:
 
1. Personal Information:
  • Your full name, social security number, and date of birth
  • Your spouse's full name, social security  number, and date of birth
  • Ex-spouse's (if any) full name, social security number, and amount of alimony paid, Form 8332 (if any)
  • Your chidren's full name, social security number, and date of birth
  • Other adults in your home, social security number, date of birth, and income information
  • Copy of your 2009 income tax returns both federal and state
2. Income Information:
  • Salary and Wage Income, Form W-2
  • Interest Income, Form 1099-INT
  • Dividend Income, Form 1099-DIV, 1099-OID
  • Self-Employment Income, Form 1099-MISC, Schedule K-1, Income not recorded in 1099 (with records of expense and assets)
  • Retirement Income, Pension/IRA/annuity income/social security income, Form 1099-R, 1099-SSA
  • Rental Income (with rental asset date in service & cost and rental expense records)
  • Sales of stock or other property, Form 1099-B, 1099-S ( with cost and acquired date)
  • Scholarships and fellowships
  • State tax refund, Form 1099-G
  • Unemployment, Form 1099-G
  • Alimony received from ex-spouse, his/her name, social security number, and amount
  • Gambling income, Form W-2G (with expense records)
  • Health care reimbursements, Form 1099-SA or 1099-LTC
  • Jury duty income
  • Hobby income (with expense records)
  • Prizes and awards
  • Others
3. Deductions and Credit Information:
  • Mortgage statement, Form 1098
  • State and local tax paid (other than wage withholding)
  • Real estate property tax paid
  • Personal property tax paid, such as DMV registration fee
  • Vehicle sales tax paid (with invoice)
  • Purchase of a new home (with final escrow paper)
  • Childcare expense (with the provider's name, social security namber, or ID number)
  • Tuition payments, Form 1098-T, and education related expenses
  • Cash donation (with proof, such as receipts or cancel checks)
  • Non-cash donation, such as clothes and toys ( with records)
  • Total amount paid for health insurance, doctors, dentists, hospital, and medicals
  • Mileage driven for charitable or medical purposes (with records)
  • Amount paid for your 2009 income tax preparation
  • Expenses related to your investments
  • Employment-related expenses that not been reimbursed by your employer, such as travel, tools, uniforms, dues, etc.
  • Job-hunting expenses
  • Amount contributed for 2010 IRA
  • Certain qualified energy improvements or equipments to principal residence
  • Vehicle for business use information, such as total miles, business miles, gas, insurance, license, etc.
  • Office in home information
  • Others
4. Refund Information:
  • If you want direct deposit, bring a void check for your checking/saving account number and bank routing number
